Do you take insurance?

Yes! I am in network for the following insurance organizations:

Although I am in network with the organizations above, I highly recommend contacting your insurance company to verify I am in network. My name may be listed as Mackenzie Fox.

I am NOT Medicare certified and cannot accept Medicare-based insurances, even if in the networks listed above.

What should I know about using my insurance for therapy services?

You are responsible for any balance that is not covered by insurance. You can use your Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay your balance.

It is always a good idea to contact your insurance company prior to starting therapy services. You can contact them using the phone number listed on the back of your insurance card. Helpful questions to ask your insurance company include:

Is Reflective Counseling or Mackenzie Fox in network with my insurance plan?

Does my insurance plan cover behavioral health services?

Do I have a co-pay or co-insurance for therapy services? (If needed, you can supply the therapy service code 90837 with modifier HO. The claim amounts I submit to insurance are the same as my self-pay rates listed below.)

Do I have a deductible that applies to therapy services? If so, what is my current deductible balance?

Does my insurance plan have a limit of how many sessions per calendar year I can claim? If so, what is that limit?

 

What is your self-pay rate for mental health counseling?

Intake session (usually lasting 60 to 90 minutes): $160

Individual 55-minute session: $135

Fees are payable by cash, check, or credit card at the time of service.

What is a Good Faith Estimate?

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

Where are you located?

My office is at 205 5th Ave S, Ste 320 in La Crosse, WI. My office is in the Exchange Building, which is located to the left of the Children’s Museum downtown La Crosse. I am on the 3rd floor (there is an elevator!) towards the end of the hallway. There is a waiting room inside my suite door. You may wait there until I am ready for your session.

 

Do you offer telehealth (virtual) sessions?

Yes! Telehealth, or virtual sessions, are a great option if you live too far from my office. (Please note, you must live in the state of Wisconsin for me to meet with you, as required by my state license). It is also a great back up option in case of bad weather or if someone is feeling ill. For virtual sessions, I use a platform called doxy.me. Doxy.me is a simple, user-friendly program that is HIPPA-compliant. It is accessible from desktop computers, laptops, tablets, and smartphones. You do not have to download anything or create an account to use it. Simply click the link below to enter my virtual waiting room and type in your name. I will connect with you from there at your appointment time.

How do I make an appointment?

If you are a current client, you may log in to the Client Portal to find & request an open appointment time. If you are a new client, please visit the Contact page to request an initial consultation. Do I need to fill out paperwork prior to my first session?

Yes, it is preferred! I use an electronic health record system called Simple Practice. Once we have scheduled your intake session, you will receive an email from Simple Practice with a request sign and/or to fill out the required forms, including demographic information, an informed consent, my privacy policy, my practice policies, and a questionnaire about what brings you to counseling. Forms can be accessed through computer, laptop, tablet, or smartphone.

 

What can I expect at my first counseling session?

Everyone is nervous for their first counseling session! At your first session, we will review the intake paperwork you have completed, and I can address any questions you have about the forms. Then, we will start to get to know each other. The intake session might feel like I am asking question after question; however, I am doing the best I can to understand you and your circumstances from a holistic and comprehensive perspective. By the end of the session, my hope is that we are able to identify 2-3 specific goals that will guide our time together. The success of counseling is very much influenced by the fit between the counselor and the client, so you are also encouraged to ask any questions you may have of me and how I view treatment. How many counseling sessions will I need?

Unfortunately, there is no standard answer for this. The number of sessions, and how often they occur, is up to you. What I have found works best is meeting weekly for one month, as we are getting to know each other, and then determine how often will feel best for you from there.

 

What is your cancellation policy?

A 24-hour notice of cancellation is preferred. I recognize that sometimes life happens without notice, so I trust that you will inform me of any cancellation as soon as you are able.
